Griep en het influenzavirus
Influenza virussen zijn RNA virussen behorende tot de familie Orthomyxoviridae en zijn opgedeeld in 3 verschillende types (influenza A, influenza B en influenza C).
Influenzavirussen worden overgedragen door de lucht via infectieuze druppels en direct contact. Mensen met griep zijn reeds besmettelijk van 1 dag voor het verschijnen van symptomen tot 5-7 dagen erna (ref). De incubatietijd bedraagt 0,5 tot 1,5 dagen (ref).
Influenzavirussen veroorzaken influenza of griep, een aandoening die wordt gekenmerkt door het plots opzetten van koorts, hoofdpijn, spierpijn en hoest, keelpijn, rhinitis. Soms zijn ook buikpijn, overgeven, misselijkheid en diarree geassocieerd met influenza infectie.
Het influenza virus is wereldwijd een belangrijke oorzaak van morbiditeit en mortaliteit. Gemiddeld wordt globaal jaarlijks 5 tot 10% van de volwassenen en 20% tot 30% van de kinderen besmet met influenza, met als resultaat 3-5 miljoen gevallen van ernstige ziekte en 1 miljoen overlijdens per jaar (ref 1;ref 2). Vooral bij jonge kinderen, bejaarden en specifieke risicogroepen (b.v.b. hoogzwangere vrouwen, mensen met chronische ziekte of immunodeficiëntie) is er kans op ernstige verwikkelingen (ref)
Jaarlijks doet zich een epidemie voor, gewoonlijk tijdens de winter. Ongeveer vier keer per eeuw duikt een nieuw influenza virus op dat een pandemie veroorzaakt. De ernst van de epidemieën en pandemieën is enerzijds afhankelijk van kenmerken van het circulerend virus en anderzijds afhankelijk van de kwetsbaarheid van de getroffen populatie.
Organisatie van de griepsurveillance in België
Omdat griep jaarlijks een hoge tol aan morbiditeit en mortaliteit eist, een belangrijke maatschappelijke impact kan hebben en door vaccinatie enigszins vermijdbaar is, werd zowel op nationaal als op Europees vlak een uitgebreid surveillancesysteem voor influenza uitgebouwd.
Sciensano werd in 2006 door het Interministrieel Influenza Comité verantwoordelijk gesteld voor het uitvoeren van alle surveillance activiteiten inzake influenza in België. De internationale surveillance wordt gecoördineerd door ECDC en WHO.
Naast epidemiologisch onderzoek omvatten de nationale surveillance activiteiten ook virologisch onderzoek van ernstige griepgevallen. Het epidemiologisch onderzoek wordt uitgevoerd door de eenheid Epidemiologie van infectieziekten en Gezondheidszorgonderzoek binnen de Directie Epidemiologie en Volksgezondheid. Het virologisch onderzoek wordt gevoerd door het virologisch laboratorium van Sciensano, dat erkend is als nationaal referentiecentrum (NRC) voor influenza.
De surveillance geeft informatie over:
duur en intensiteit van de jaarlijkse epidemie van seizoensgriep circulerende virussen (typering en antigene en genetische karakterisatie) impact en ernst resistentie tegen antivirale middelen vaccinale effectiviteit

Het voorkomen van griep in ons land wordt opgevolgd door middel van informatie afkomstig uit verschillende bronnen.
NRC Influenza
Het influenzalaboratorium van Sciensano is reeds sinds 1977 actief en werd in 2010 officieel erkend als het Nationale Referentiecentrum (NRC) voor Influenza. Het NRC influenza test klinische stalen afkomstig die enerzijds verzameld worden in het kader van de surveillance van griepaal syndroom door het netwerk van huisartsenpeilpraktijken en de surveillance van ernstige acute respiratoire infecties door een netwerk van peilziekenhuizen (SARI) , en anderzijds stalen die verzameld worden buiten de context van surveillance. Deze laatste stalen worden afgenomen bij patiënten met een ernstig griepaal syndroom die bepaalde specifieke kenmerken vertonen (ARDS, extracorporele membraan oxygenatie, overlijden, vermoeden van resistentie tegen antivirale middelen en bij een terugkeer uit het buitenland).

Netwerk van huisartsenpeilpraktijken
Het peilnetwerk van huisartsen registreert sinds 2007 continu de consultaties bij de huisarts voor griepaal syndroom en acute luchtweginfecties. Voor elke episode worden leeftijdsgroep, vaccinatiestatus, uitkomst en hospitalisatie genoteerd. Dit verloopt in nauwe samenwerking met het NRC influenza, dat voor een (willekeurige) subgroep van de geregistreerde gevallen een klinisch staal microbiologisch onderzoekt op influenza. Bij deze subgroep worden eveneens bijkomende klinische gegevens geregistreerd (symptomen, risicofactoren en co-morbiditeit, vaccinatie, behandeling en indicatoren van ernst).
Netwerk van peilziekenhuizen
Een peilnetwerk van 6 ziekenhuizen registreert sinds het respiratoir seizoen 2011-2012 alle episodes van ernstige acute respiratoire infecties (SARI) die plaatsvinden in de periode van verhoogde griepactiviteit. De registratie start wanneer de eerste tekenen van circulatie van influenzavirussen worden gedetecteerd door de NRC Influenza en stopt 3 weken nadat de incidentie van griepaal syndroom (geregistreerd door het netwerk van huisartsenpeilpraktijken) terug onder de epidemische drempel valt. Het gaat om 6 ziekenhuizen met hoge ICU activiteit. Voor elke episode worden tijdens het ziekenhuisverblijf verschillende parameters geregistreerd over demografische kenmerken van de patiënt, symptomen, risicofactoren en co-morbiditeit, vaccinatie, behandeling, ernst en uitkomst. Naast deze registratie van klinische gegevens, wordt bij elke patiënt ook een nasopharyngeaal staal afgenomen dat op influenza getest wordt door het NRC influenza.
Netwerk van peillaboratoria
Sinds 1983 coördineert Sciensano een peilnetwerk van laboratoria voor microbiologie die op vrijwillige basis de microbiologische diagnoses van 36 pathogenen registreren. Het netwerk registreert sedert 1996 alle positieve testen voor influenza.
Wanneer is er een griepepidemie?
De epidemische drempel is het minimum aantal huisartsconsultaties voor griepachtige klachten per 100.000 inwoners per week dat nodig is om officieel van een epidemie te kunnen spreken.
De epidemie begint van zodra de epidemische drempel twee weken na elkaar bereikt/overschreden wordt in het netwerk van huisartsenpeilpraktijken en er minstens 20% van de door Sciensano geanalyseerde virale stalen positief zijn voor het griepvirus. De epidemie eindigt op het moment dat het aantal huisartsconsultaties omwille van griepklachten terug onder de epidemische drempel duikt.
De overdracht van griep inperken
Iedereen kan de overdracht van griep helpen inperken. Volg ook het onderstaande basisadvies:
- Raadpleeg zo snel mogelijk uw huisarts als u griepsymptomen heeft en volg zijn/haar aanbevelingen
- Blijf thuis en vermijd zoveel mogelijk overbodig contact om de verspreiding van de ziekte te voorkomen
- Was uw handen goed en frequent met water en zeep
- Hoest bij voorkeur in een papieren zakdoek en werp hem na gebruik meteen weg; hoest tegen uw arm -niet in uw handen- als u geen zakdoek heeft.

Aanbevelingen vaccinatieDe Hoge Gezondheidsraad, een wetenschappelijk adviesorgaan van de Federale Overheidsdienst Volksgezondheid, Veiligheid van de Voedselketen en Leefmilieu, formuleert jaarlijks de risicogroepen voor griep en de aanbevelingen voor het vaccin.
Het griepvaccin wordt idealiter tussen midden oktober en midden november toegediend. Zolang het virus nog niet massaal circuleert, is het opportuun om zich nog steeds te laten vaccineren. De griepprik biedt immers bescherming binnen een termijn van 10 �� 15 dagen volgend op de injectie.
Het WIV raadt risicopersonen die nog niet zijn gevaccineerd aan om alsnog contact op te nemen met hun huisarts, die hun individuele situatie zal beoordelen. Het gaat vooral om:
· Personen van 65 jaar en ouder
· zwangere vrouwen vanaf het 2e trimester van hun zwangerschap
· personen met een chronische ziekte
· verzorgend personeel
Samenstelling vaccin
De samenstelling van de griepvaccins wordt elk jaar aangepast op basis van de aanbevelingen van de Wereldgezondheidsorganisatie (WGO). De quadrivalente vaccins voor het griepseizoen 2018-2019 bevatten de virale stammen die vorige winter het meest in omloop waren omdat de kans groot is dat zij ook dit jaar aanwezig zullen zijn.
Dit seizoen bevat het quadrivalente vaccin de volgende virussen:
- A/Michigan/45/2015 (H1N1)pdm9
- A/Singapore/INFIMH-16-0019/2016 (H3N2)
- B/Colorado/06/2017 of een verwante stam (B-Victoria-lijn)

La grippe et le virus influenza
Les virus influenza sont des virus à ARN, subdivisés en trois types différents (influenza A, influenza B et influenza C). Ce sont principalement les types a et B qui ont une importance clinique chez l’homme.
Les virus influenza se transmettent par l’air, via des gouttelettes infectieuses, et par contact direct. Les personnes grippées sont déjà contagieuses 1 jour avant l’apparition des symptômes et le restent jusqu’à 5-7 jours par après (ref). Le temps d’incubation varie de 0,5 à 1,5 jour (réf).
Les virus influenza sont responsables de l'influenza ou la grippe, une affection caractérisée par certains des symptômes suivants: l’apparition brutale de fièvre, céphalées, douleurs musculaires, toux, maux de gorge, rhinite,... Parfois, l’infection par l’influenza s’accompagne également de douleurs abdominales, de vomissements, de nausées et de diarrhée.
À l’échelle mondiale, le virus influenza est une cause importante de morbidité et mortalité. Globalement, en moyenne, 5 à 10 % des adultes et 20 à 30 % des enfants sont infectés par l’influenza chaque année, avec 3-5 millions de cas de maladie sévère et 1 million de décès par an (réf 1; réf 2). Chez les patients jeunes ou âgés et dans les groupes à risque sp��cifiques (p. ex. les femmes en fin de grossesse, les personnes souffrant d���une maladie chronique ou d’immunodéficience), il existe un risque de complications sévères (réf).
Chaque année, on observe une épidémie, habituellement durant l’hiver. Environ quatre fois par siècle, un nouveau virus influenza fait son apparition et provoque une pandémie. La sévérité des épidémies et des pandémies dépend d’une part des caractéristiques des virus circulants, et d’autre part de la vulnérabilité de la population touchée.
Organisation de la surveillance de grippe en Belgique
Étant donné que la grippe entraîne chaque année une morbidité et mortalité élevée, qu’elle peut avoir un impact social important et qu’elle est en partie évitable grâce à un vaccin, un vaste système de surveillance de l’influenza a été mis sur pied, tant au plan national qu’au plan européen et international.
En 2006, le Comité interministériel Influenza a confié la responsabilité de la réalisation de toutes les activités de surveillance de l’influenza en Belgique à Sciensano. La surveillance internationale est coordonnée par l'ECDC et l’OMS.
Outre les études épidémiologiques, les activités de surveillance nationale englobent également l’étude virologique des cas de grippe sévères, notamment les ces de grippe sévère. Les études épidémiologiques sont effectuées par l'unité Epidémiologie des Maladies infectieuses et Etudes des soins de santé au sein de la Direction Epidémiologie et Santé Publique. Les études virologiques sont effectuées par le laboratoire de virologie de Sciensano, qui est reconnu comme Centre National de Référence (CNR) pour l’Influenza.
La surveillance vise à suivre :
la durée et l’intensité de l’épidémie annuelle de grippe saisonnière les virus circulants (types, sous-types, caractéristiques antigéniques et génétiques) l’impact et la sévérité la résistance aux antiviraux l'effectivité vaccinale
Sources de données
CNR Influenza
Le laboratoire influenza de Sciensano est actif depuis 1977, et a été officiellement reconnu comme Centre National de Référence (CNR) pour l’Influenza en 2010. Le CNR pour l’Influenza teste des échantillons cliniques collectés d’une part dans le cadre de la surveillance des syndromes grippaux, par le réseau de médecins généralistes vigies et de la surveillance des infections respiratoires aiguës sévères (SARI) par un réseau d’hôpitaux vigies, et d’autre part, il teste des échantillons prélevés en dehors du contexte d’une surveillance. Ces derniers échantillons sont collectés chez des patients souffrant d’un syndrome grippal sévère, qui présentent certaines caractéristiques spécifiques (ARDS, oxygénation par membrane extracorporelle, décès, suspicion de résistance aux antiviraux et lors de cas suspect de grippe aviaire ou autres virus respiratoires chez des patients de retour de pays à risque). site web du CNR Influenza
Réseau de médecins généralistes vigies
Depuis 2007, le réseau sentinelle des médecins généralistes enregistre en continu les consultations en médecine générale pour les syndromes grippaux et les infections aiguës des voies respiratoires. Pour chaque épisode, on consigne le groupe d’âge, le statut vaccinal, l’issue et les hospitalisations. Ceci se passe en collaboration étroite avec le CNR pour l’Influenza, qui effectue une recherche virologique de l’influenza, au départ d’un échantillon clinique, chez un sous-groupe (arbitraire) de cas enregistr��s. Dans ce sous-groupe, on enregistre également des données cliniques supplémentaires (symptômes, facteurs de risque et comorbidités, vaccination, traitement et indicateurs de sévérité).
Réseau d'hôpitaux vigies
Depuis la saison 2011-2012, un réseau de six hôpitaux enregistre tous les épisodes d’infections respiratoires aiguës sévères (SARI) qui se produisent au cours de la période d’activité grippale élevée. L’enregistrement débute dès que les premiers signes de circulation de virus influenza sont détectés par le CNR pour l’Influenza, et prend fin au moins 3 semaines après que l’incidence de syndromes grippaux (enregistrés par le réseau de médecins généralistes vigies) soit repassée sous le seuil épidémique. Il s’agit de six hôpitaux disposant d���un service de soins intensifs très actif. Pour chaque épisode, on enregistre – durant l’hospitalisation – différents paramètres au sujet des caractéristiques démographiques du patient, des symptômes, des facteurs de risque et des comorbidités, de la vaccination, du traitement, de la sévérité et de l’issue. Outre cet enregistrement de données cliniques, un échantillon nasopharyngé est prélevé chez chaque patient, en vue d’une recherche de l’influenza qui sera effectuée par le CNR pour l’Influenza.
Réseau de laboratoires vigies
Depuis 1983, Sciensano coordonne un réseau de laboratoires vigies de microbiologie qui enregistrent sur une base volontaire les diagnostics microbiologiques de 36 pathogènes. Il s’agit plus précisément d’infections des voies respiratoires, du système nerveux central, d���infections gastro-intestinales, d’infections importées, d’infections sexuellement transmissibles et de zoonoses. Depuis 1996, le réseau enregistre tous les tests positifs pour l’influenza.
Quand est-ce qu'il y a une épidémie de grippe?
Le seuil épidémique se définit comme le nombre minimal de consultations pour syndromes grippaux par semaine et par 100.000 habitants, enregistrés par le réseau des médecins vigies.
Chaque année, le Centre européen de prévention et de contrôle des maladies (ECDC) calcule, pour chaque pays distinct, le seuil épidémique, sur la base des chiffres des cinq dernières années. Cette année, le seuil est fixé à 141 consultations pour 100 000 habitants en Belgique.
L’épidémie est déclarée lorsque le seuil épidémique est atteint ou dépassé pendant deux semaines consécutives et lorsqu’au moins 20 % des échantillons viraux analysés par Sciensano se révèlent positifs pour les virus de la grippe. L’épidémie prend fin au moment où le nombre de consultations pour des sympt��mes grippaux redescend sous le seuil épidémique.

Recommandations de vaccination
Le Conseil Supérieur de la Santé, un organe d'avis scientifique du service public fédéral Santé publique, sécurité de la chaîne alimentaire et environnement, formule chaque année la liste des groupes à risque pour la grippe ainsi que les recommandations liées au vaccin en se basant sur l'évidence susmentionnée et sur la revue de la littérature.
Idéalement, le vaccin contre la grippe doit être administré entre mi-octobre et mi-novembre. Tant que le virus ne circule pas massivement, il est toujours opportun de se faire vacciner, le vaccin offrant une protection dans un délai de 10 à 15 jours suivant l’injection.
Sciensano conseille aux personnes à risque non encore vaccinées de prendre contact le plus rapidement possible avec leur généraliste. Celui-ci décidera de les vacciner ou non, en fonction de leur état de santé et/ou de leur situation personnelle. Il s’agit principalement :
- des personnes âgées de 65 ans et plus
- des femmes enceintes à partir du 2ème trimestre de grossesse
- des personnes souffrant d’une maladie chronique (insuffisance rénale, pulmonaire, cardiaque, hépatique, etc.)
- du personnel soignant
Composition du vaccin
La composition des vaccins est revue chaque année sur la base des recommandations de l’Organisation mondiale de la Santé (OMS). Pour la saison grippale 2018-2019, les vaccins quadrivalents contiennent les souches virales choisies en février 2018 sur la base des souches qui ont dominé lors de la saison précédente.

Because flu yearly causes high morbidity and mortality, can have an important societal impact and is vaccine-preventable, both at national and at European level a comprehensive surveillance system for Influenza was created.
In 2006, the Interdepartmental Influenza Committee appointed Sciensano (then WIV-ISP) as the institution responsible for influenza surveillance activities in Belgium. International surveillance is coordinated by ECDC and WHO.
In addition to epidemiological studies, national surveillance activities also include virological characterization of severe influenza cases. Epidemiological studies are conducted by the units Epidemiology of infectious diseases and Health services research within the directorate Epidemiology and public health. Virological studies are carried out by the Virology laboratory of Sciensano, which is recognized as the National Reference Center Influenza.
The surveillance provides information on
• Duration and intensity of the annual seasonal flu epidemic
• Circulating viruses (typing, subtyping, antigenic and genetic characterization)
• Impact and severity of the annual seasonal flu epidemic
• Antiviral resistance
• Vaccine effectiveness
Data Sources
National reference Centre Influenza
The Sciensano influenza laboratory has been active since 1977, and has been formally recognized as the National Reference Centre (NRC) for influenza since 2010. The NRC Influenza tests clinical specimens collected on the one hand as part of the surveillance of influenza-like Illness (ILI), collected by the sentinel network of general practitioners and of the surveillance of severe acute respiratory infections (SARI) collected by a sentinel network of hospitals, and on the other hand, it tests samples taken outside the context of surveillance. The latter samples are collected in patients with severe flu-like symptoms with specific characteristics (ARDS, extracorporeal membrane oxygenation, death, suspected antiviral resistance and suspected cases of avian influenza or other emerging respiratory viruses).
Sentinel network of general practitioners
Since 2007, the sentinel network of general practitioners has continuously recorded the general medicine consultations for influenza and acute respiratory tract infections. For each episode age group, vaccination status, outcome and hospitalization are recorded. In a subset of these patients, a clinical sample is collected and virologically tested by the NRC Influenza. From this subgroup, additional clinical data (symptoms, risk factors and co-morbidities, vaccination, treatment and severity indicators) are also recorded.
Sentinel network of hospitals
Since the 2011-2012 respiratory season, a network of six hospitals has recorded all episodes of hospitalized severe acute respiratory infections (SARI) that occur during the period of high influenza activity. The surveillance starts as soon as the first signs of influenza virus circulation are detected by the NRC Influenza, and ends at least 3 weeks after the incidence of influenza-like syndromes (Sentinel Network of General Practitioners) again dropped below the epidemic threshold. The participating hospitals have an active intensive care service. For each episode, the patient's demographic characteristics, symptoms, risk factors and co-morbidities, vaccination status, treatment, severity and clinical outcome are registered during the hospital stay. In addition to this clinical data recording, a nasopharyngeal sample is collected from each patient and virologically tested by the NRC Influenza.
Sentinel network of laboratories
Since 1983, Sciensano has been coordinating a network of microbiology lookout Laboratories that voluntarily register microbiological diagnostics of 36 pathogens. These include respiratory tract infections, central nervous system, gastro-intestinal infections, imported infections, sexually transmitted infections and zoonoses.
